About us

A brief story about our why

We are women’s health physicians dedicated to engaging pregnant and postpartum populations across California. We believe in delivering high-touch care to address adverse outcomes that disproportionately impact birthing people of color, particularly in the fourth trimester & postpartum period. This call to action resonates with us at Bridge Prenatal and our mission of truly assessing social determinants of health within the prenatal and postpartum populations. We believe that effective solutions must build trust with patients, improve accessibility, and address inequities of care. Shifting the site of care for components of the patient care journey outside of the hospital or clinic setting, creates an opportunity to improve adherence to protocols and recommendations that elevate care quality & patient satisfaction.

We strive to offer an innovative approach to enhancing the care management of high-risk pregnant and postpartum patients through a comprehensive, patient-centric program focused on improving clinical outcomes through early and frequent contact with eligible high-risk members.
