We accept pregnant and postpartum patients up to 12 months who are covered by CHG, Molina, and IEHP.

Referrals are simple. You can use our referral form and we can provide pre-filled copies for ease. Referrals can be submitted by secure fax, email, or through your clinic’s usual process.

Once we receive a referral, our team submits the ECM authorization, contacts the patient directly, explains our services, and schedules their first visit with a Care Companion RN. You will receive the physician-signed clinical chart notes via e-fax.

Our team provides comprehensive care including:

• Nurse visits for vitals, initial screenings, and thorough medical assessment
• Social worker/Case Manager support for mental well-being and community resources
• Lactation and doula support
• In-home/office biophysical profiles – the only specialty within our program that needs to be requested by a physician on the referral form. Sonograms are not patient-requested.

We offer close monitoring with more frequent visits, remote support, and coordination with specialists. Our services are documented into Medi-Cal care plans to ensure continuity of care.

We provide weekly faxes of your patients’ clinical chart notes and contact your team right away if any concerns arise. Our goal is always to supplement the excellent care you provide, not replace it.
