The Bridge Prenatal team is dedicated to providing the highest quality of care for every patient across San Diego County and the Inland Empire. Bridge Prenatal does not replace your scheduled medical visits with your physician or midwife – instead, we serve as an additional layer of personalized, integrated support throughout your pregnancy and postpartum journey. We offer flexible scheduling and prioritize culturally competent approaches to care. Care is delivered by qualified clinicians in convenient locations, including your home. Bilingual clinicians are available.

We include social determinants of health and mental health screening assessments to address and prevent chronic illness and morbidity, with earlier detection at the forefront. Wrap-around services at the home not only enable deeper family and community engagement but also enhance safety and comfort.

Experience an additional layer of prenatal and postnatal care with flexible scheduling, convenient home visits, and telehealth options. Our comprehensive prenatal care services include nurse check-ups, social worker visits, lactation consultations, doula support, prenatal monitoring, and essential childbirth education and preparation – all tailored to your individual needs and delivered where it’s most comfortable for you.

Postpartum Services

For up to one year after delivery, we provide nurse check-ups, visits from social workers, lactation counseling, and more to ensure you receive the necessary support for a healthy postpartum journey. Our maternity care doesn’t stop at birth – we’re with you every step of the way. Gain confidence in caring for your newborn with our informative baby care education sessions, covering topics such as feeding, sleep, vaccines, and infant safety.

What Is Enhanced Care Management?

Enhanced Care Management (ECM) is a Medi-Cal benefit designed to provide whole-person, coordinated care for individuals with complex health and social needs – including pregnant and postpartum women. Bridge Prenatal is a ECM provider serving California families across San Diego County and the Inland Empire.

Through our Enhanced Care Management program, your dedicated care team helps coordinate your medical appointments, connect you with community resources, and provide the ongoing support you need for a healthy pregnancy and postpartum recovery. ECM services are available at no cost to eligible Medi-Cal members and are designed to remove the barriers that prevent many families from accessing the maternal health care they deserve.

Enhanced Care Management (ECM) is a Medi-Cal program funded through the state of California that provides coordinated, whole-person support for pregnant and postpartum women with complex needs. At Bridge Prenatal, our ECM services include dedicated care coordination, home visits, mental health screenings, and connections to community resources – all delivered by qualified clinicians across San Diego County and the Inland Empire.

We bring trusted maternal care directly to you. Home-based services include Care Companion RN visits, Resource Companion & Community Connector support, lactation consulting, remote sonography, and doula visits across San Diego County and the Inland Empire.

Yes, you can decide which visits you would like in person at home and which ones you prefer through telehealth. Our team works with you to design a schedule that fits your needs and comfort level. Many families choose in-home visits for physical check-ups, lactation support, and recovery care, while telehealth is often used for follow-ups, education, and counseling.

In addition to medical care, we offer visits with our Resource Team, who help connect you with resources for housing, transportation, food, WIC, baby clothes, diapers, car seats, medically tailored meals, furniture, groceries, and much more. We may also connect you to therapy to make sure you’re feeling fully supported throughout your pregnancy and postpartum period.
