We offer flexible scheduling and prioritize culturally competent approaches to care. Care is delivered by qualified clinicians, in convenient locations, including the home. Bilingual clinicians are available.
We include social determinants of health and mental health screening assessments to address and prevent chronic illness and morbidity, with earlier detection at the forefront. Wrap-around services at the home not only enable deeper family and community engagement but also enhance safety and comfort.
Yes, you can decide which visits you would like in person at home and which ones you prefer through telehealth. Our team works with you to design a schedule that fits your needs and comfort level. Many families choose in-home visits for physical check-ups, lactation support, and recovery care, while telehealth is often used for follow-ups, education, and counseling. This way, you receive hands-on care when it is most helpful and the convenience of virtual visits when that works best for you.
In addition to medical care, we offer visits with our Resource Team who help connect you with resources for housing, transportation, food, WIC, baby clothes, diapers, and connect you to therapy to make sure you’re feeling supported throughout your pregnancy and postpartum period.
We deliver compassionate postpartum support, including a Care Companion RN follow-up, screenings for your mental health, hands-on lactation guidance, and ongoing maternal care for you after your baby is born for a full year.
